Do other people see my medical notes or files?
After your Health Assessment a summary of the assessment & copy of a Health Care Action Plan (making sure that things that need to happen for you to have a healthy life do happen ) is sent to your GP, your Social Worker & Children Looked After Nurses . A copy of the Health Care Plan is sent to you, to your carer & perhaps other people who are named on the Plan that need to do something to help your health .
You should be told about where these things are sent before you have the Health assessment in case you are not happy about some people seeing this information & then you can discuss those concerns about sharing the summary & Plan with the Doctor or Nurse who is doing your Health assessment .Usually other people are not allowed to see your medical notes or files. Anything you say to your doctor or any other health professional is confidential unless they are worried that you may be in danger of harm. In that case they would have to pass on information to your social worker but they should tell you if they do this. As you get older you will take more responsibility for your health.
